Reach each item carefully and rate yourself according to the specific performance indicators. Check the column corresponding to your level of performance in the accompanying Summary Rating Sheet (SRS)- PASS Form B-2. For example, in item No. 1 A. *Formulate/Adopts objectives of lesson plan when all were met, check under column 10. Do the same for the rest of the items. Enter your score under RATING.
